online. After each class is over, download and complete this
form. You may turn it in via email or print it and send via fax,
or you may send it by US Postal Service Mail. Forms must arrive
in our offices no later than 5 business days after last day of
each course is completed to be used in course survey.

Networking is the best tool anyone can have to help manage their career,
and we encourage our students to attend events, meetings, and functions of professional
organizations, and keep in touch with classmates and business associates. Please
